Patients in the Merlin (CP-GEP) High-Risk group were around 20 times more likely to experience a recurrence, compared to the Merlin Low-Risk group. Their 5-year and 10-year RFS was 37.5% for both endpoints. Additionally, High-Risk patients showed significantly increased rates of distant metastasis and melanoma-specific death, reinforcing the clinical value of CP-GEP in identifying patients at risk for recurrence. While SLNB remains the standard for melanoma staging, many patients classified as low-risk based on traditional methods still experience relapse or even death. The CP-GEP model, which combines gene expression data with clinical factors age and tumor thickness, effectively identified two distinct risk groups among patients who did not undergo SLNB. Merlin CP-GEP test delivers high clinical utility – Study prospectively confirms precise risk stratification, optimizing SLNB referral recommendations. The MERLIN_001 trial is the largest prospective, blinded study, to evaluate the Merlin CP-GEP test, which is the only commercially avialable GEP test that combines clinical, pathological, and gene expression data into a single algorithm to predict SLN positivity. The study was conducted at 9 renowned US hospitals, which are also top-ranking oncology academic centers. The study's findings validate the test's superior predictive accuracy compared to traditional clinical and pathological factors, which currently guide sentinel lymph node biopsy (SLNB) decisions in melanoma. A total of 1,802 patients were included for whom SLN biopsy material was available. CP-GEP achieved an assay success rate of 97.7%, thus a total of 1,761 patients could be enrolled. The National Comprehensive Cancer Network (NCCN) clinical guidelines recommend SLNB surgery for patients with >10% risk of nodal metastases. This cohort had a 17.6% overall SLNB positivity rate, suggesting effective referral of patients based on current factors. However, 82.4% of patients had a negative sentinel node suggesting no therapeutic benefit from the sentinel node operation. How did Merlin CP-GEP test provide more precise risk stratification, allowing physicians to further optimize SLNB referral decisions and personalize treatment plans? Key findings from the MERLIN_001 study: In this cohort a total of 31 T1a patients with adverse features were referred for SLNB surgery. Merlin identified 80.6% as Low Risk for nodal metastases at a negative predictive value (NPV) of 100%. These patients could consider forgoing SLNB surgery as their risk of nodal metastases is close to 0%. Six patients were identified as Merlin High Risk. Their risk of metastases was with 16.7% well above the >10% guideline threshold, therefore these patients should be referred for surgery. Among the 479 T1b patients, Merlin test identified 67.6% as Low Risk for nodal metastases with a 94.8% NPV. These patients could consider forgoing SLNB surgery as their risk of metastases is close to 5%. Additionally, T1b patients with a Merlin High Risk label had a 16.1% SLNB positivity rate and should be referred for surgery. The study's findings are particularly significant for T2a melanoma patients. This patient subgroup had an SLN positivity rate of 15.5%. Merlin test identified 36.6% as Low Risk at an NPV of 91.9%, reducing their metastatic risk below the 10% threshold for SLNB surgery recommendation. Reclassification of these T2a patients gives them access to more personalized treatment options. The Merlin CP-GEP test also demonstrated exceptional performance in other subgroups, particularly in head and neck melanoma patients. SLNB surgery in this group poses unique challenges due to complex lymphatic drainage and vital anatomical structures. Merlin test identified 43.3% as Low Risk for nodal metastases with an NPV of 95.1%. These patients could consider forgoing SLNB surgery as their risk of metastases is below 5%. Based on the principles of scientification, standardization, transparency and institutionalization with adherence to the international guidelines writing standards and guiding principles for the formulation/revision of clinical diagnosis and treatment guidelines in China, experts adopt evidence-based medicine methods, keep updated on the newest progress, focus on the diagnostic principles, immunohistochemistry and molecular pathology, establish quality evaluation and evidence summary, utilize the GRADE system and Delphi method in combination with multidisciplinary expert meetings, draft, modify and finalize this version of practice guideline for the histopathological diagnosis of mesothelioma. 间皮瘤（mesothelioma）在中国的发病率有逐年升高的趋势［1］。精准的病理学诊断和规范化病理学检查报告对间皮瘤临床治疗和患者预后判断至关重要。为建立间皮瘤组织病理学诊断规范，中国研究型医院学会病理学专业委员会和上海市抗癌协会肿瘤病理专业委员会组织了来自国内不同医院和不同领域的专家，本着科学化、规范化、透明化和制度化的原则，围绕间皮瘤最新进展，聚焦间皮瘤病理学诊断原则、免疫组织化学标志物和分子病理学检测，编撰间皮瘤组织病理学诊断指南，以期为病理科医师在临床实践中提供借鉴证据，提高中国间皮瘤病理学诊断的规范化水平。 1 指南制定方法 1.1  指南目的 进一步提高中国间皮瘤组织病理学诊断水平和病理学检查报告规范性，为病理科医师在临床实践中提供借鉴证据，为临床治疗和患者预后判断提供重要信息。 1.2  目标人群和使用人群 本指南的目标人群为间皮瘤患者，使用人群为从事间皮瘤病理学诊断和辅助检测的医务工作者。 1.3  指南制订采用方法 遵循国际指南编写标准和中华医学会《中国制订/修订临床诊疗指南的指导原则》（2022版）［2］。 1.4  启动与计划 本指南确定为《间皮瘤组织病理学诊断临床实践指南（2025年版）》，在 国 际 实 践指南注册与透明化平台（http://www.guidelines-registry. cn）进行中英文双语注册，注册号为PREPARE-2024CN672，参考卫生保健实践指南的报告条目（the Reporting Items for practice Guidelines in HealThcare，RIGHT）撰写指南计划书［3］，制作指南制订流程图（图1）。 图1  指南制订流程图 1.5  成立指南工作组 组建首席专家兼利益冲突管理委员会、指导专家组、秘书组、文献检索和证据收集组、临床问题遴选和确定专家组、证据评价组、共识组、多学科专家组、执笔组和外审组等工作组，涵盖病理、影像、肿瘤内科、胸外科和流行病学与卫生统计学等多领域专家，具有间皮瘤流行病学或临床诊治经验。 1.6  管理利益冲突 所有参与指南制定小组成员均已声明所有潜在经济或非经济利益冲突，并填写《间皮瘤组织病理学诊断临床实践指南（2025年版）》利益冲突声明表（参见计划书）。本指南利益冲突管理委员会根据利益冲突声明表评估专家成员利益冲突程度，采取相应的管理措施。 1.7  证据检索 检索的英文数据库包括PubMed、Embase、 Web of Science和 Cochrane Library，中文数据库包括中国知网、万方和中国生物医学文献数据库。检索时限均为建库至2024年10月31日。 英文文献检索词主要包括“malignant mesothelioma”、“mesothelioma”、“mesothelioma in situ”、“pathology”、“pathological diagnosis”、 “ immunohistochemistry”、“BAP1”、 “MTAP”、“molecular pathology ”、 “cytogenetics”、“molecular genetics”、“genomic analysis”、“genomic landscape”、“CDKN2A”、“PD-L1”、“nivolumab”、 “ipilimumab”、“consensus”、“guideline”、“update”和“dataset”。中文文献检索词主要包括 “恶性间皮瘤”、“间皮瘤”、“临床病理”、“病理诊断”、“免疫组织化学”、 “BAP1基因”、“MTAP基因”、 “CDKN2A（p16）基因”、 “分子病理”、 “分子遗传学”、“PD-L1”、“纳武利尤单抗”、“伊匹木单抗”和“免疫治疗”等。采用逻辑组配等检索方法。 1.8  证据和推荐等级 本指南根据循证医学证据等级，参照证据推荐分级评估、制定和评价系统（Grading of Recommendations，Assessment，Development and Evaluation，GRADE）对循证医学证据质量和专家推荐的强度进行分级，其中证据级别包括：A级证据为高度确定性，B级证据为中等确定性，C级证据确定性有限，D级证据则确定性极低（参见附录B）；推荐等级包括：推荐等级包括：强，中，弱（参见计划书）。 1.9  共识达成方法及标准 采用德尔菲问卷调查法和共识会议法相结合，就间皮瘤相关“陈述”进行专家函询。投票设置“非常同意”、“基本同意”、“不确定”、“不同意”和“非常不同意”5个选项，专家对每条“陈述”提出修改意见。每次调查结束后，根据专家反馈意见对“陈述”进行修改或增补。 1.10  临床问题汇总和推荐意见形成 对全国28个省、市、自治区多家医院的病理科医师进行调研，131名专家参与问卷填写，收集了21条推荐建议。通过在线会议形式，由指南专家委员会和秘书组对临床问题重要性调研结果进行讨论，将收集到的临床问题进行去重、修改、合并和汇总，删除6条陈述，根据间皮瘤病理学诊断相关的临床问题进行归纳总结15条陈述，其中同意率为100%共6条，强推荐为14条，中推荐1条，弱推荐0条。 1.11  指南撰写、修改和定稿 执笔组参考RIGHT起草《间皮瘤组织病理学诊断临床实践指南（2025年版）》初稿，召开专家会议审定初稿，并请外审专家评议，综合审定意见进行修改，形成终稿。 1.12    指南的传播、实施、评价与更新 本指南通过：① 在本刊上公开发表本指南，并可免费获取；② 在相关学术会议中对本指南进行解读；③ 通过媒体、公众号等进行推广；④ 在中国部分省市自治区组织本指南的推广专场会议，确保基层及以上单位病理科医师充分了解并提高间皮瘤病理学诊断水平。对指南进行定期或不定期调研，评价指南实施效果。 本指南有效期为5年。随着间皮瘤临床病理学证据更新和完善，指南将在颁布后第5年启动新版指南修订工作，第6年发布新版指南。  2 间皮瘤组织病理学诊断 2.1  间皮瘤诊断名称的使用 既往常采用恶性间皮瘤（malignant mesothelioma）这一名称，如恶性胸膜间皮瘤（malignant pleural mesothelioma）、恶性腹膜间皮瘤（malignant peritoneal mesothelioma）、上皮样恶性间皮瘤（epithelioid malignant mesothelioma）、双相性恶性间皮瘤（biphasic malignant mesothelioma）或肉瘤样恶性间皮瘤（sarcomatoid malignant mesothelioma），现推荐统一采用间皮瘤来诊断，不再加用“恶性”前缀，如胸膜间皮瘤（pleural mesothelioma，PM）、腹膜间皮瘤（peritoneal mesothelioma ，PeM）、上皮样间皮瘤（epithelioid mesothelioma，EM）、双相性间皮瘤（biphasic mesothelioma，BM）和肉瘤样间皮瘤（sarcomatoid mesothelioma，SM）等（证据级别：A；推荐等级：中）。 背景与证据： 既往采用间皮瘤名称的病变除了恶性间皮瘤外，还包括囊性或多囊性间皮瘤（cystic or multicystic mesothelioma，CM/MCM）和高分化乳头状间皮瘤（well differentiated papillary mesothelioma，WDPM）［4-5］，易引起误解。CM/MCM属于良性瘤样病变，WDPM属于良性或恶性潜能未定肿瘤（benign or neoplasm of uncertain malignant potential），为避免与恶性间皮瘤相混淆，CM/MCM已改称间皮包涵囊肿（mesothelial inclusion cyst，MIC）［6］，WDPM则改名高分化乳头状间皮肿瘤（well differentiated papillary mesothelial tumor，WDPMT）［7］。因此，第5版世界卫生组织（World Health Organization，WHO）肿瘤分类（包括胸部肿瘤、女性生殖道肿瘤及泌尿和男生殖肿瘤）推荐直接采用间皮瘤，不再加用“恶性”前缀［8-9］。 2.2  间皮瘤的疾病编码及其临床意义 根据间皮瘤解剖部位和组织形态学将间皮瘤诊断转化为ICD-O和ICD-11编码，便于对间皮瘤进行诊断和统计管理（证据级别：A；推荐等级：强）。 背景与证据： ICD-O由WHO制定［10］，全称为《国际肿瘤疾病分类（International Classification of Diseases for Oncology）》。ICD-O编码在癌症登记和研究中发挥重要作用。通过将实际疾病诊断转化为编码形式，便于标准化和国际化，不仅有助于监测癌症疾病发病率和死亡率，为制定治疗策略和预防措施提供数据支持，而且有助于公共卫生专业人员更好地分析癌症疾病分布、趋势和影响因素。  ICD-11也由WHO制定［11］，全称为《国际疾病分类第十一修订版（International Classification of Diseases 11th Revision，ICD-11）》。ICD-11根据疾病病因、部位、病理学和临床表现等特征，将疾病转化成编码，进行有序分类，利于系统记录、分析、解释、比较全球不同国家或地区、不同时期死亡率和发病率等情况。ICD-11不仅为临床医师提供了更准确、更全面的疾病诊断，也实现了数据可存储、检索、分析和应用，为医学研究提供了全球统一疾病分类标准，为健康管理和研究提供了便利，为政策制定和采取干预措施提供科学依据。 间皮瘤的ICD-O和ICD-11编码参见表1。 2.3  石棉纤维接触史与间皮瘤诊断 石棉纤维是间皮瘤的重要致病因素［12］，但是否有石棉纤维接触史不能成为确诊或除外间皮瘤的依据（证据级别：A；推荐等级：强）。 背景与证据： 间皮瘤发病机制较复杂，除石棉纤维外，还包括非石棉性矿物纤维（如毛沸石、氟代-浅闪石和羟硅铁锰石等）、慢性感染和放射线照射（治疗性或职业性）等多种因素［13-14］。新材料碳纳米管（carbon nanotube）可能具有与石棉纤维相似致病毒性等［15］。少数间皮瘤发生于BAP1肿瘤易感综合征（BAP-1 tumor predisposition syndrome）家族，由BAP1胚系突变/缺失所致［16］。胸膜间皮瘤也可为自发性，无特殊致病因素［17］。心包间皮瘤、睾丸旁间皮瘤和多数女性腹膜间皮瘤（包括ALK和EWSR1重排亚型）与石棉纤维无相关性［18-19］。 2.4  间皮瘤的生长方式及其临床意义 正确区分弥漫性间皮瘤（diffus e mesothelioma，DM）和局限性间皮瘤（localized mesothelioma，LM）对临床治疗和患者预后判断具有重要意义（证据级别：A；推荐等级：强）。 背景与证据： 绝大多数间皮瘤（99%）呈弥漫性生长，又称DM；极少数间皮瘤（1%）呈局限性生长，又称LM。LM的诊断需结合临床和影像学［20］，见表2。迄今为止，文献上累计报道120多例。大多数病例发生于胸膜，部分病例发生于腹膜［21］，少数病例发生于心包、肝内、脾脏、胰腺、胃、肠系膜、输卵管和睾丸鞘膜等处［22-25］。LM的镜下形态、免疫表型和分子改变与DM相似［26］，但患者预后明显好于DM，中位生存期为80.4～134.0个月［22］。 2.5  间皮瘤组织病理学诊断 间皮瘤组织病理学诊断需包括间皮瘤组织学亚型及与预后相关的细胞形态、排列结构和间质改变（证据级别：A；推荐等级：强）。 背景与证据： 组织病理学诊断是间皮瘤诊断金标准。间皮瘤组织学亚型包括上皮样、肉瘤样和双相性［27-28］，并显示不同的细胞形态、排列结构和间质改变，参见表3，其中一些形态学特征与患者预后相关（参见后述）。 EM占 PM的 60%～70% ，占PeM的80%～90%。经典型EM由多边形、立方形或卵圆形细胞组成，细胞质丰富，嗜伊红色或淡染。低核级EM中的瘤细胞核显示轻度异型性（图2A），细胞核大小和形态一致，核仁不明显或见小核仁；中核级EM中的瘤细胞核呈中等大小，显示中度异型性（图2B），核大小和形状可有不一致，可见明显核仁；高核级EM中的瘤细胞显示重度异型性，细胞核大小和形状明显不一致，核染色质粗，核仁明显，常见核分裂象（包括病理性）（图2C）［29-30］。部分EM中的瘤细胞呈横纹肌样、蜕膜细胞样、小细胞性、透明细胞样、印戒细胞样、淋巴组织细胞样和多形性，其中具有横纹肌样和多形性形态者具有较高侵袭性，患者预后不佳［31-32］。EM有多种排列结构，且数种结构可见于同一肿瘤内。常见结构包括管状-乳头状、梁状和实性，少见结构包括腺瘤样（微囊性）和微乳头状等（图2D～H）。显示管状-乳头状、梁状和腺瘤样结构者预后相对较好，显示实性（占比≥50%）和微乳头状结构者预后相对较差［33-34］。20%～30%的EM伴有坏死（图2I），与患者预后不佳相关［35］。部分EM间质呈黏液样，瘤细胞呈小簇状或单个散在分布于疏松黏液样基质内（图3A）。黏液样区域≥50%，实性区域＜50%者预后较好［36］。淋巴组织细胞样间皮瘤由类似组织细胞的多边形瘤细胞和间质内大量炎症细胞浸润组成（图3B），后者多为CD8阳性T淋巴细胞。部分EM间质伴有明显纤维化或胶原化。少数EM中可见沙砾体，偶可伴有泡沫样组织细胞反应。 SM占PM的15%，占PeM的5%～10%。SM预后较EM差［37-38］。经典型SM由束状、交织状、片状或杂乱状排列梭形细胞组成（图3C），瘤细胞显示程度不等异型性，核分裂象易见（包括病理性），常伴有坏死。肿瘤常浸润脂肪组织或邻近结构（包括肺实质）。具有淋巴组织细胞样形态特征的SM预后相对较好［39］。促纤维性或促结缔组织增生性间皮瘤（desmoplastic mesothelioma）是SM的一种特殊亚型，由排列杂乱的轻度异型梭形细胞和大量伴有玻璃样变的纤维间质组成，后者在肿瘤内占50%以上（图3D），预后不佳［40］。活检标本中如见到促纤维性间质，宜诊断为SM伴有促纤维形态（sarcomatoid mesothelioma with desmoplastic features）（图3E）［28］。伴有异源性分化（with heterogeneous differentiation）的SM显示骨肉瘤、软骨肉瘤或横纹肌肉瘤等异源性分化（图3F）［41］。具有移行形态的间皮瘤（transitional mesothelioma）其瘤细胞形态介于上皮样和肉瘤样之间（图3G），常呈片状生长［42］。转录组研究和网状纤维染色支持移行性间皮瘤为SM且预后不佳［43-44］。SM也可显示多形性形态。 BM占PM的15%～30% ，占PeM的10%～20%，是心包间皮瘤最常见类型。BM由上皮样成分和肉瘤样成分混合组成［45］，两种成分之间或分界清楚（图3H），或相互混杂（图3I）。手术切除标本中每种成分均需≥10%，活检标本无此要求。BM的生物学行为介于EM和SM之间。肉瘤样成分＞50%者预后较差，故在术后或活组织病理学检查报告中应注明上皮样成分和肉瘤样成分在BM中的占比［27-28］。 图2 mesothelioma，PEM）进行分级不仅可对预后进行分层，而且对临床治疗策略的选择也具有重要参考价值（证据级别：B；推荐等级：强）。 背景与证据： 2012年Kadota等［29］通过多变量分析发现核异型性和核分裂象是PEM独立的预后因子，并据此设计了三分层核分级（three-tiered nuclear grading），将PEM分为三个预后组，研究发现三个预后组在生存率方面的差异有统计学意义。该分级系统简单、实用且能更好地对预后进行分层，并得到后续研究验证［30，46-48］，其中Rosen等［35］研究还发现肿瘤是否伴有坏死也与预后相关，进而将坏死与三分层核分级相结合，形成双分层肿瘤分级（two-tiered tumour grading）（表4），得到专家共识（expert consensus）、美国病理学家学会（College of American Pathologists，CAP）、WHO、国际间皮瘤学会（International Mesothelioma Interest Group，iMiG）和欧洲临床肿瘤学会指导委员会（ESMO Guidelines Committee）等强推荐［8，27-28，30，49-50］。该分级系统除可对PEM患者的预后进行分层外，对临床治疗策略的选择也具有重要参考价值［51］。目前，该分级系统主要用于PEM临床实践。PM活检和切除标本配对研究显示，两种标本类型在评估核级和坏死上的一致性分别达75%和81%，间皮瘤分级有助于活检后的治疗选择［52］。对腹膜EM以及不同组织学亚型间皮瘤的预后分层新近也有研究报道［53-55］，但鉴于PeM的流行病学及生物学行为与PM有所不同，临床治疗也不同，故如何对PeM进行分级评估仍有待达成共识。其他分级系统包括澳大利亚学者结合年龄、组织学类型、坏死、核分裂象、核异型性和BAP1表达情况的间皮瘤加权分级方案（mesothelioma-weighted grading scheme，MWGS）以及预后列线图［56-57］。 PEM分级的不足之处在于，三级核分级源于病例研究并得到证实，但双分层肿瘤分级最初源于专家共识，在临床应用之前未经病例研究验证，尽管后续报道显示具有预后价值［47-48］。此外，细胞形态（如多形性或横纹肌样）和排列结构（如实性或微乳头状）和分子改变（如CDKN2A缺失）等与患者预后相关的因素也并未纳入分级中。新近有学者报道，Ki-67增殖指数与PEM的预后相关［58］。Ki-67增殖指数0%～4%、5%～9%以及≥10%相当于核分裂象计数评分的1分、2分和3分。由于核级评估在可重复性上存在一定问题，采用核分裂象计数和Ki-67增殖指数（类似神经内分泌肿瘤中的分级）能否替代核级评估对PEM进行分级有待于更多研究。 2.7  间皮瘤免疫组织化学标记 联合采用一组间皮标志物（mesothelial markers）辅助间皮瘤诊断。如为肉瘤样型，加做GATA3。活检标本加做BAP1（证据级别：A；推荐等级：强）。 背景与证据： EM采用间皮标志物包括calretinin、D2-40、WT1和CK5/6［8，27-28，59］。需要注意的是，calretinin、D2-40和CK5/6等在一部分癌中也可有灶性或斑片状表达。新抗体HEG1（克隆号SKM9-2，膜染色）和SPARC（兔多克隆抗体，细胞质染色）对EM有较高特异性和敏感性［60-61］。EM失表达BAP1（细胞核）（55%～70%）（图4A）、MTAP（细胞质）（35%～65%）和Merlin（细胞膜/细胞质）（35%～41%）［59］。具有横纹肌样形态间皮瘤可失表达SMARCB1（INI1）［62］。ALK重排间皮瘤可弥漫表达ALK［63］。少数腹膜EM表达 PAX8［64］，但不表达ER和PR。 SM以表达AE1/AE3为主［8，37-38］。辅助诊断SM时联合采用AE1/AE3、D2-40、calretinin、WT1和GATA3。Calretinin和WT1在SM中的表达或为灶性阳性或为阴性。D2-40在SM中常呈细胞质着色。SM常弥漫表达GATA3（图4B）［65-66］。部分SM（10%～36%）失表达BAP1，但常失表达MTAP（61%～83%）［59，67］。 BM肿瘤内上皮样成分和肉瘤样成分的免疫表型分别与EM和SM相似。BM常失表达BAP1 （47%～77%）、MTAP（0～60%）和Merlin（0～70%）［59，68-69］。 2.8  间皮瘤的PD-L1检测 根据临床需要加做PD-L1检测（证据级别：A；推荐等级：强）。 背景与证据： CheckMate 743研究显示，对不可手术切除间皮瘤采用纳武利尤单抗联合伊匹木单抗方案一线治疗效果优于化疗［70］。PD-L1免疫组织化学（TPS≥1%）可预测免疫治疗效果。总体上，PD-L1在非EM（SM和BM）中的表达率高于EM。PD-L1在EM、SM和BM中的阳性表达率分别为10%～49%、9%～67%和22%～100%（图4C）［71-73］。 图4  不同部位间皮瘤的标志物检测 A: 检测BAP1、CDKN2A、MTAP、NF2等基因缺失或失活突变有助于间皮瘤的诊断与鉴别诊断，并为患者的预后预测提供参考（证据级别：A；推荐等级：强）。 背景与证据： 大多数间皮瘤以体细胞突变和拷贝数改变为特征，包括BAP1、CDKN2A、MTAP、NF2、TP53、LATS1/2和SETD2等，并因部位和组织学亚型而异［59，74-76］：① BAP1编码去泛素化酶，在细胞周期调节、细胞分化、细胞死亡和DNA修复反应中发挥作用。BAP1在60%的PM和70%的PeM中失活，包括错义突变、截短突变和剪接点突变。BAP1突变是间皮瘤发生中的早期事件，多见于EM，并为原位间皮瘤中最常见的分子改变。BAP1的分子改变在EM中比在SM中更常见［28］。BAP1胚系突变引发BAP1肿瘤易感综合征［16］。② CDKN2A（9p21.3，编码p16）缺失在间皮瘤的发生中属于早期事件。70%的PM（40%～70%EM和BM以及90%SM）和10%～15%的PeM显示CDKN2A纯合性缺失（图4D）［77］。75%～90%的CDKN2A缺失的间皮瘤常伴有邻近的CDKN2B与MTAP（9q21.3）基因共缺失［78］，可通过MTAP免疫组织化学标志物（42-T，Santa Cruz）或联合采用MTAP和p16免疫组织化学标志物替代CDKN2A FISH检测［79］。③ NF2（22q12.2，编码Merlin）失活包括截短突变、移码突变、点突变或缺失，在间皮瘤发生中属于晚期事件，相比EM（0～40%），NF2突变或缺失多见于BM和SM（0～70%）［80］。NF2突变或缺失可通过测序或FISH检测，也可通过Merlin免疫组织化学标志物（克隆号D1D8或D3S3W）检测［81］。BAP1和CDKN2A基因突变检测对间皮瘤诊断的特异性并非100%，但有助于与良性间皮病变的鉴别诊断［82］，并与患者预后相关（参见后述）。此外，少数透明细胞EM显示VHL（3p25.3）突变［83-84］，多发生于腹膜，少数位于胸膜。 少数间皮瘤涉及基因重排，包括：① ALK重排，主要见于儿童腹膜间皮瘤，并多见于女性。ALK伴侣基因包括STRN、ATG16L1和TPM1［85］。② EWSR1重排，多见于儿童和青年腹膜间皮瘤。EWSR1伴侣基因包括YY1和ATF1［86-88］。基因重排及其融合基因可通过FISH和（或）二代测序（next generation sequencing，NGS）检测。 2.10  原位间皮瘤的病理学诊断 原位间皮瘤（mesothelioma in situ，MIS）的诊断需要结合临床、影像学、免疫组织化学标志物和（或）分子检测（证据级别：A；推荐等级：强）。 背景与证据： MIS是一种在胸膜或腹膜表面生长的非浸润性间皮肿瘤，最早由Whitaker等［89］于1992年提出。Churg等［90］于2018年首先报道了MIS实例，发生于反复胸腔积液且不消退患者，影像学或胸腔镜检查无肿瘤证据，但活检标本显示胸膜表面有单层扁平状或略呈乳头状间皮增生，加做免疫组织化学和（或）FISH显示BAP1表达缺失和（或）CDKN2A纯合性缺失，并至少在1年时间无浸润性间皮瘤发生。后续报道佐证了MIS这一病变实体［91-92］，不仅发生于胸膜，少数病例发生于腹膜和睾丸鞘膜［93-94］。MIS的诊断标准为：① 反复性胸水或腹水；② 影像学或胸腔镜/腹腔镜检查显示无肿块证据；③ 胸膜或腹膜表面单层扁平或立方形间皮细胞，伴有或不伴有异型性（图4E）；或表面有小或复杂性乳头状增生，可类似WDPTM［95］；或表面有小结节，伴有中度至重度异型，但无胸膜下或腹膜下浸润灶；④ 免疫组织化学标志物检测显示BAP1失表达（图4F），和（或）MTAP免疫组织化学失表达，或FISH检测显示CDKN2A（p16）纯合性缺失；⑤ 必要时经多学科讨论。MIS可进展为浸润性间皮瘤［91，96-97］，但潜伏期相对较长，随访显示（中位随访时间5年），70%的MIS进展为浸润性间皮瘤［91］，故MIS一经诊断后，其处理应提交多学科讨论。 2.11  间皮瘤的诊断路径 间皮瘤的病理学诊断需结合临床、镜下形态、免疫组织化学标志物和分子检测（证据级别：A；推荐等级：强）。 背景与证据： 胸/腹膜病变确认为间皮增生病变后，如临床有明确肿块形成，显微镜下显示恶性形态特征（如浸润性生长），可直接诊断间皮瘤，常规辅助检测可包括BAP1和（或）MTAP免疫组织化学。对形态上诊断困难者，可先行BAP1和（或）MTAP免疫组织化学检测，如有表达缺失，可诊断为间皮瘤，如无缺失，加做分子检测（FISH或基因测序）检测CDKN2A，如有缺失，可诊断为间皮瘤，如无缺失，则诊断为非典型性间皮增生，对活检病例备注必要时再复查，对手术病例备注定期复查和随诊。对临床上无肿块形成，但患者有反复胸腹水者，活检标本显示胸/腹膜表面病变者应加做BAP1和（或）MTAP免疫组织化学和（或）FISH检测CDKN2A基因，如有BAP1和（或）MTAP表达缺失和（或）CDKN2A基因缺失，则诊断为原位间皮瘤［28］。间皮瘤诊断路径参见图5。 图5  间皮增生病变诊断路径 IHC: 免疫组织化学检测; FISH: 荧光原位杂交。 2.12  间皮瘤的鉴别诊断 基于临床病理学特征结合辅助检测排除类似疾病对间皮瘤诊治具有重要意义（证据级别：A；推荐等级：强）。 背景与证据： 鉴别诊断是间皮瘤病理学诊断中的重要环节。 与肺腺癌相鉴别时采用一组上皮标志物（epithelial markers），包括CEA、MOC-31、BerEP4和claudin-4［8，27，59，98］。需要注意的是，10%～15%的间皮瘤也可斑片状表达MOC-31和BerEP4［59］。与其他上皮性肿瘤鉴别时采用的免疫组织化学标志物因瘤种而异。 EM需鉴别的病种包括：① 反应性间皮增生，除镜下形态外，联合采用BAP1、MTAP标志物和（或）FISH检测CDKN2A（p16）有助于区分间皮瘤和良性间皮增生［82］。② WDPMT，EM的局部区域可类似WDPMT，但其他区域显示典型EM形态，包括明确的浸润性生长。与EM不同，WDPMT表达EMA不佳，但可表达L1CAM［99］，多数病例（60%～95%）还可表达PAX8［5］，BAP1表达无缺失，FISH检测显示无CDKN2A缺失。因TRAF7突变也可见于局限性胸膜间皮瘤和腹膜弥漫性间皮瘤［26，100］，不能单凭TRAF7突变鉴别WDPMT和间皮瘤。③ 累犯胸膜的上皮肿瘤可致胸膜增厚而类似间皮瘤，也称假间皮瘤样癌（pseudomesotheliomatous carcinoma）［101］，包括肺腺癌、鳞状细胞癌以及胸膜转移性癌。联合采用上皮标志物，包括claudin-4、MOC-31、 BerEP4、TTF-1、Napsin A、p63和p40，有助于鉴别诊断。SPARC对实体性EM与实体性肺腺癌及低分化鳞状细胞癌的鉴别有一定帮助［61］，但不能鉴别SM和含有梭形细胞成分的多形性癌。转移性癌如转移性乳腺癌和肾细胞癌等可加用相应标志物，包括mammaglobin、GCDFP-15、GATA3、CD10、PAX2、PAX8、P504S（AMACR）和RCC等（表5）。需注意的是，40%的EM也可表达 GATA3［102］。④ 腹腔浆液性腺癌，除镜下形态外，联合采用calretinin、BerEP4、MOC-31、ER、PR、PAX2和PAX8等标志物帮助鉴别。⑤ 实性乳头状间皮肿瘤（solid papillary mesothelial tumor），多发生于女性腹膜，境界清楚，由成片、索样或乳头状排列的间皮细胞组成，偶见核沟，BAP1表达无缺失，FISH检测CDKN2A无缺失，生物学上呈惰性［103］。⑥ NR4A3重排间皮肿瘤（NR4A3-rearranged mesothelial tumor），发生于腹膜，镜下以腺瘤样（微囊性）/筛状结构为特征，常局限性或部分表达calretinin和D2-40，分子检测显示NR4A3重排，伴侣基因多为EWSR1，少数病例为CITED2和NIPBL［104］。⑦ 其他，包括SWI/SNF复合物缺失性未分化癌/肿瘤（BRG1/INI1缺失）与具有横纹肌样形态的上皮样间皮瘤相鉴别等。 SM需鉴别的病种包括：① 肉瘤样癌，GATA3有助于肉瘤样间皮瘤SM与肉瘤样癌的鉴别诊断［65-66］。此外，部分肺肉瘤样癌可表达高相对分子质量CK、p63、p40和TTF1，分子检测显示MET14号外显子跳突变。② 梭形细胞滑膜肉瘤，瘤细胞形态一致，无明显多形性，常灶性表达上皮标志物（AE1/AE3和EMA），弥漫核表达SS18-SSX，FISH检测显示SS18基因重排。 ③ 孤立性纤维性肿瘤，主要与促纤维性间皮瘤相鉴别。联合采用AE1/AE3、CD34和STAT6有助于鉴别诊断。④ 机化性胸膜炎，AE1/AE3可显示机化性胸膜炎中的区带现象。此外，机化性胸膜炎不表达PD-L1，而肉瘤样间皮瘤常呈阳性表达［105］。⑤ 其他梭形细胞肉瘤，包括恶性周围神经鞘膜瘤（失表达H3K27Me3）等。 BM需鉴别的病种包括：① 伴有纤维性间质的EM可与BM相混淆。纤维性间质无明显异型性，BAP1表达无缺失，但可表达α-SMA。② 双相性滑膜肉瘤，梭形细胞成分中的瘤细胞形态一致，无明显多形性。瘤细胞弥漫性核表达SS18-SSX。FISH检测显示SS18基因重排。③ 癌肉瘤，本质为肉瘤样癌，其中癌成分可表达上皮标记，根据不同部位可分别表达TTF1、Napsin A、ER、PAX8、CEA、CDX2、claudin-4、BerEP4和MOC-31等标志物。 2.13  间皮瘤患者预后相关病理学因素 与间皮瘤患者预后相关的病理学因素包括组织学亚型、细胞形态、排列结构、间质改变、坏死、分级（胸膜EM）以及BAP1/MTAP表达和CDKN2A缺失状况（证据级别：A；推荐等级：强）。 背景与证据： 间皮瘤的组织学亚型与患者预后相关，EM预后相对较好，BM居中，SM预后较差［106］，但不同亚型内在临床生物学行为和预后上存在差异。MTAP失表达、p16表达＜1%和FISH检测显示CDKN2A缺失预后不佳［75，79，107］，与之相反，BAP1失表达和BAP1胚系突变者预后相对较好［108-109］。与间皮瘤预后相关的病理学参数参见表6。 全基因组分析根据转录组数据，确定了4个不同的预后分子簇，包括肉瘤样、上皮样、双相上皮样和双相肉瘤样，与总生存期和上皮-间质转化的程度相关［74］。基于整合的基因组、转录组和表观基因组数据，TCGA队列研究确定了4个不同的预后亚型［75］。预后不良分子簇具有上皮—间质转化高评分、间皮素低mRNA表达、T辅助细胞2（Th2）细胞特征高评分以及富集LATS2突变和CDKN2A纯合缺失。Blum等［110］通过结合转录组、甲基化组和miRNome分析，证明胸膜间皮瘤具有不同的上皮样和肉瘤样成分（E评分和S评分）。评分与肿瘤微环境-血管之间存在相关性［111］。S评分与T淋巴细胞、单核细胞、成纤维细胞、内皮细胞和PD-L1高表达相关，E评分与NK细胞浸润、补体途径和VISTA过表达相关，这些结果与PD-L1蛋白表达、肉瘤样间皮瘤、预后不良和间质淋巴细胞浸润相一致［112-113］。 2.14  间皮瘤的规范化病理学检测报告 规范化的间皮瘤病理学检测报告需包括标本类型、肿瘤部位、组织学类型、ICD-O和ICD-11编码、核级（仅适用于PEM）、组织排列结构、瘤细胞形态、间质形态、病理学分期（适用于胸膜间皮瘤）和相关辅助检测等（证据级别：A；推荐等级：强）。 背景与证据： 病理学检测报告是一份重要的医疗文件。规范化的间皮瘤病理学检测报告对间皮瘤诊断的准确性、完整性和一致性至关重要，可为临床治疗策略的选择和预后判断提供重要的信息。国际癌症报告合作组织（International Collaboration on Cancer Reporting，ICCR）也发表了间皮瘤推荐病理学诊断报告［114］（线上查阅网址为：https://www.iccr-cancer.org/datasets/published-datasets/thorax/ mesothelioma）以及CAP癌症报告模板（弥漫性胸膜间皮瘤v5.0.0.0，线上查阅网址为：https://www.cap.org/protocols-and-guidelines/cancer-reporting-tools/cancer-protocol-templates），可供参考和查阅。需要说明的是，病理学分期pTNM目前仅适用于胸膜间皮瘤［115-116］，心包间皮瘤、腹膜间皮瘤和睾丸旁间皮瘤尚无共识。对腹膜间皮瘤也有采用腹腔癌症指数（peritoneal cancer index，PCI）评估病变范围者［117-118］。间皮瘤的规范化病理学检测报告参见表7。 2.15  间皮瘤的人工智能辅助诊断 人工智能辅助诊断对间皮瘤的组织学分型有一定的帮助，但不能替代病理科医师的工作（证据级别：A；推荐等级：强）。 背景与证据： 间皮瘤的组织学分型与间皮瘤患者的预后相关，经典EM和SM的诊断对大多数病理科医师来说并不困难，但在部分间皮瘤的组织学分型上包括专家在内也存在不一致，特别是针对移行性间皮瘤和多形性间皮瘤的诊断。此外，BM中的肉瘤样成分也与患者预后密切相关，如何准确地评估肉瘤样成分的占比也非常重要。一项人工智能模型（CHOWDER）识别移行性间皮瘤的准确率达94%［43］，但尚不能完全替代病理科医师［119］。人工智能辅助间皮瘤组织学分型、肿瘤分级、分子生物学检测结果预判和预测患者预后等方面有待于进一步研究［120］。 3 总结与展望 精准的病理学诊断和规范的病理学报告不仅为间皮瘤的临床治疗提供了参考和指导，相关的组织学分层和辅助检测也为预后判断提供了重要信息。完整和全面的报告在为前瞻性研究提供数据工具的同时，也为回顾性分析提供了真实可靠的数据。随着分子研究的深入开展，一些以分子改变为特征的间皮瘤亚型也为临床进行相关的靶向治疗提供了循证医学证据。对间皮瘤及其亚型开展的深度学习也有助于人工智能辅助诊断工具的开发和应用，最终为患者带来获益。
