A spectrum of congenital, inherited, or acquired abnormalities in BLOOD VESSELS that can adversely affect the normal blood flow in ARTERIES or VEINS. Most are congenital defects such as abnormal communications between blood vessels (fistula), shunting of arterial blood directly into veins bypassing the CAPILLARIES (arteriovenous malformations), formation of large dilated blood blood-filled vessels (cavernous angioma), and swollen capillaries (capillary telangiectases). In rare cases, vascular malformations can result from trauma or diseases.

Evaluation of the Efficacy and Safety of Anti-angiogenic Therapy with Intravenous Bevacizumab in Patients with Symptomatic Cerebral Arteriovenous Malformations

Brain arteriovenous malformations (AVMs) are responsible for hemorrhagic strokes, particularly in children and young adults. They can also be responsible for chronic neurological disorders: motor or sensory deficits, disturbances of higher functions, epilepsy or disabling headaches. The management of brain AVMs is complex and requires a multidisciplinary approach in an expert center. Available therapies include endovascular embolization, neurosurgical resection and/or radiosurgery. These procedures carry a risk of neurological complications, and are reserved for small AVMs located at a distance from highly functional cerebral structures. To date, no drug therapy is recommended if interventional treatment is not possible.
Several studies on resected brain AVM tissue have demonstrated that these malformations are the site of significant evolutionary inflammatory and neo-angiogenesis processes. Other studies have specifically shown that VEGF (vascular endothelial growth factor) levels are increased in AVMs. More recently, a pre-clinical study showed that anti-angiogenic treatment with Bevacizumab reduced vascular proliferation within AVMs in mice. Finally, a Phase II clinical trial in patients with Rendu-Osler disease (a genetic vascular disorder characterized by recurrent epistaxis, cutaneous telangiectasia and the presence of visceral AVMs) showed a clinical benefit of IV Bevacizumab on the symptomatology of these vascular malformations, with a reduction in the risk of hemorrhage and the extent of hepatic arteriovenous shunts. A randomized Phase III trial is currently underway (NCT03227263) to assess the efficacy of IV Bevacizumab in Rendu-Osler disease.
The aim of our study is to assess the efficacy of IV Bevacizumab on the disabling symptoms associated with symptomatic brain AVMs.

Clinical Trial for the Evaluation of the Accuracy of Augmented Reality Based Neuronavigation System

The goal of this clinical trial is to test augmented reality (AR) based neuronavigation system in surgeries for patients of brain neoplasm or cerebral vascular disease. The main questions it aims to answer are:
• AR based neuronavigation system can achieve accuracy that is not inferior to conventional intraoperative navigation system.
Participants will participate the study after informed consent. When participants undergo surgery for their brain tumor, we will set up 2 types of neuronavigation, conventional navigation system and developed AR based neuronavigation system. Surgeon will plan and conduct surgery based on only conventional navigation system, but 3D errors at several selected points between two types of navigation will be measured and analyzed.

Liquid Biopsies for the Detection of Somatic Mutations in Brain Arteriovenous Malformations

"Personalized medicine has revolutionized patient care, particularly in oncology. Brain arteriovenous malformations (bAVMs) are abnormal vessels located on the surface of the brain or within the brain parenchyma, causing abnormal communication between arterial and venous networks, without the interposition of the capillary bed. The main risk of these malformations is rupture, leading to intracranial bleeding, which can cause severe sequelae or even death. bAVMs (except those of clearly identified genetic origin [< 5%], such as mutations associated with Rendu-Osler disease) have long been considered non-genetic in origin.
However, somatic genetic mutations activating the RAS/RAF/MEK/ERK (MAPK) signaling pathway have recently been identified in surgical specimens of bAVMs. Additionally, targeted inhibition of this pathway is effective in treating these malformations in animals and appears to be effective in extracranial arteriovenous malformations, particularly superficial ones.
Next-generation sequencing of circulating DNA on liquid biopsies is a promising and minimally invasive approach to studying the presence of mutations in arteriovenous malformations.
The treatment of a bAVM aims to obliterate the malformation to prevent or avoid the risk of hemorrhage. It may involve several therapeutic modalities: microsurgery, endovascular embolization, and radiosurgery. These treatments can be combined, and microsurgery is often preceded by pre-surgical embolization, aimed at reducing the hemorrhagic risk of the intervention. However, these are invasive treatments, not without risk.
The identification of mutations through liquid biopsies could enable the development of non-invasive targeted therapies against these bAVMs.
This research aims to identify somatic genetic mutations activating the MAPK signaling pathway in bAVMs. These mutations have already been identified in surgical specimens. This research aims to evaluate the diagnostic performances of liquid biopsies (detection of genetic mutations in blood samples, i.e., circulating DNA), with the gold standard being the detection of the same mutations in surgical specimens."

Using a plastic cannula prevents arteriovenous fistula failure in early cannulation

AIMTo study the effect of early arteriovenous fistula (AVF) cannulation and the usage of plastic cannulas on short- and long-term AVF primary patency.METHODSA database of 1,127 access operations from 1 November 2009 to 1 January 2011 was retrospectively analyzed. In total, 765 AVFs used for dialysis with follow-up until 1 December 2015 were included in the study. The period between operation and the initial cannulation, termed first cannulation time (FCT), was classified into intervals of 1-2 weeks, 3-4 weeks, 5-6 weeks, and 7-12 weeks. Primary patency was the interval between AVF creation and the first re-intervention for access dysfunction or thrombosis, the time of measurement of patency or the of its abandonment.RESULTSThe AVF primary patency was 98.1% after 1 year and decreased to 80.1% after 2 years. Among patients using metal needles for the first puncture, those with an FCT of 1-2 weeks experienced significantly decreased AVF primary patency compared to other groups. Conversely, in patients using plastic cannulas at the first puncture, no significant differences were observed among groups with AVF primary patency in 6, 12, and 24 months. FCT and the application of plastic cannulas were significantly associated with primary patency at 12 and 24 months after adjusting for age, sex, hemodialysis (HD) frequency, and HD duration per session. Combination of six variables including age, sex, FCT, application of plastic cannulas, hemodialysis (HD) frequency, and HD duration per session could accurately predict 6-month primary patency with area under ROC curve of 0.89, 95% CI: 0.80-0.99.CONCLUSIONSEarly AVF cannulation, within 1-2 weeks of creation, using plastic cannulas, does not increase the risk of access failure and offers an alternative to reduce reliance on catheters.

Thoracic Intradural Extramedullary Cavernous Malformation Mimicking Meningioma

Cavernous malformation, also known as cavernoma or cavernous hemangioma, is a benign vascular malformation characterized by abnormal proliferation of capillaries that lack intervening neural tissue. Spinal intradural extramedullary cavernous malformations are exceedingly rare and, therefore, are often misdiagnosed. Here, we report a rare case of thoracic intradural extramedullary cavernous malformation that mimicked meningioma on preoperative imaging. A 77-year-old male patient presented with a two-month history of myelopathy, progressive paresthesia in the lower extremities, and difficulty walking, noted during an outpatient clinic visit. Spinal magnetic resonance imaging detected an intradural extramedullary mass lesion on the dorsal side at the Thoracic (Th) 10 vertebral level, compressing the thoracic cord. Preoperative imaging revealed a uniform T1- and T2-weighted intensity signal, suggesting meningioma. Intraoperatively, the mass appeared reddish with multiple abnormal vessels and adhered firmly to the thoracic cord. The lesion was completely resected, and postoperative pathological examination confirmed the diagnosis of cavernous malformation. The patient's preoperative symptoms improved postoperatively. Thoracic intradural extramedullary cavernous malformations are extremely rare and typically present with heterogeneous intensity signals on magnetic resonance imaging. Additionally, they can cause subarachnoid hemorrhage, emphasizing the importance of considering them in the preoperative differential diagnosis, even though imaging characteristics may be atypical. Surgical gross total resection should be considered to prevent future hemorrhage and neurological deterioration.

A Case of Normal-pressure Hydrocephalus Caused by Cauda Equina Capillary Malformation

Capillary malformations in the central nervous system, especially in the cauda equina, are rare. Although the typical symptoms are lower back pain and motor weakness in the lower extremities, we report here a rare case in which the initial symptom of a capillary malformation in the cauda equina was normal-pressure hydrocephalus. A 67-year-old woman was admitted to our hospital for close examination of hydrocephalus. Imaging studies showed no abnormalities other than an 8-mm intradural extramedullary tumor at the lumbar 2 level. Given lumbar puncture revealed a high protein level and the pressure was within the normal range, we assumed the hydrocephalus was caused by a schwannoma in the cauda equina. Within 7 days after admission, the patient's consciousness level decreased rapidly. Therefore, we immediately placed a ventriculoperitoneal shunt. Nine months after admission, the tumor increased from 8 mm to 12 mm in diameter, and tumor resection was performed. Because intraoperative neuromonitoring revealed that the tumor arose from a motor nerve, and the appearance of the tumor was not that of a typical schwannoma, we decided to remove the tumor subcapsularly. The pathological diagnosis was a capillary malformation. No neurological deficits developed in the patient postoperatively, and the high cerebrospinal fluid protein level normalized after the tumor resection. Although hydrocephalus caused by a capillary malformation in the cauda equina is relatively rare, it should be differentiated, and the strategy for its resection should be considered using the information obtained from preoperative neurological symptoms and intraoperative neuromonitoring.

阜外医院汪一波团队：甲状腺激素可促进畸形脑血管修复！

脑血管畸形会引起出血和神经功能障碍，目前尚无合适的药物治疗方式。近期，Circulation Research在线发表的中国医学科学院阜外医院汪一波团队的研究有了新突破。该研究提出了一种新的Dio2（一种将无活性的T4转化为活性T3的脱碘酶）介导的畸形脑血管适应机制，并首次阐述了甲状腺激素在脑血管畸形中的修复作用。截图来源：Circulation Research研究表明，局部Dio2过表达或系统性T3给药均能有效减少脑海绵状血管畸形和动静脉畸形小鼠的脑出血、血管细胞外基质重塑和内皮渗漏。其中T3通过激活PGC1a-Sod2/Prdx3/Gpx1信号减少活性氧产生及线粒体功能障碍，进而减少炎症浸润，改善血管异常表型。这提示，靶向甲状腺激素信号可能代表了一种潜在的脑血管疾病治疗方法。成纤维细胞是血管损伤后修复的关键细胞类型，但脑血管畸形的成纤维细胞损伤修复分子机制尚未完全阐明。该研究首先对人类脑海绵状血管畸形和脑动静脉畸形单细胞数据中成纤维细胞进行差异基因分析，结果显示甲状腺激素信号在病变样本中被显著激活，伴随DIO2表达明显升高。 ▲DIO2在畸形脑血管的成纤维细胞中表达上调随后，在脑血管畸形小鼠中进行局部性成纤维细胞特异性Dio2过表达或系统性T3给药，来评价甲状腺激素信号在脑血管畸形中的作用。结果显示，局部Dio2过表达和系统性T3给药都有效减少了脑血管畸形小鼠的脑出血发生率、细胞外基质沉积以及血管和内皮渗漏。相反，局部Dio2敲低和系统性甲巯咪唑（一种抗甲状腺激素药物）给药使模型小鼠的血管表型加重或恶化。以上结果说明，甲状腺激素信号在参与脑血管畸形病理修复中重要作用，而局部DIO2上调是一种机体的适应性机制。 ▲T3给药改善了脑血管畸形表型转录因子预测分析和分子生物学实验证实了成纤维细胞中转录因子FOXK1与DIO2启动子结合，启动DIO2转录。血管畸形成纤维细胞PI3K-Akt-mTOR信号的激活是驱动Foxk1核转位的关键因素。PI3K-Akt-mTOR信号激活促进Foxk1由细胞质向细胞核转移，促进Foxk1启动下游Dio2转录事件发生。分子生物学实验证实T3通过激活畸形血管中的Pgc1a-Sod2/Prdx3/Gpx1信号通路来改善线粒体功能和形态，减少活性氧产生和炎症浸润。▲T3改善了畸形脑血管的线粒体功能障碍综上所述，该研究在脑血管畸形中发现了由Dio2-T3-Pgc1a-Sod2/Prdx3/Gpx轴介导的血管修复机制，强调了甲状腺激素在脑血管疾病中的潜在治疗价值。局部DIO2升高和系统性T3给药均可有效减少脑血管畸形小鼠的出血、细胞外基质重塑和血管渗漏。该发现为出血干预提供了新的视角，并为针对甲状腺激素在脑血管疾病中的临床应用奠定了基础。▲DIO2升高是脑血管畸形的一种适应性机制中国医学科学院阜外医院汪一波教授为该论文通讯作者，中国医学科学院阜外医院博士生李若飞和硕士生唐宇杉为该论文共同第一作者。相关阅读《自然》子刊：长寿老人的秘密，或藏在甲状腺激素中？再次证实！《柳叶刀》子刊：低风险甲状腺癌术后无需放射性碘治疗，临床结局也不错90%的甲状腺结节无需治疗！《自然》子刊：但这3种情况应重点关注欢迎投稿：学术成果、前沿进展、临床干货等主题均可，点此了解投稿详情。参考资料[1] Li R, Tang Y, Wang H, et al. 4月29日，中国国家药监局药品审评中心（CDE）官网最新公示，上海复星医药产业发展有限公司申报的FCN-159片拟纳入突破性治疗品种，针对适应症为儿童朗格汉斯组织细胞增生症（LCH）。公开资料显示，这是复星医药研发的创新型小分子化学药物芦沃美替尼（曾用名：复迈替尼）。该产品此前已经有两项适应症的上市申请先后被CDE受理，并纳入优先审评，分别用于治疗成人树突状细胞和组织细胞肿瘤，以及治疗2岁及2岁以上儿童1型神经纤维瘤病（NF1）相关的丛状神经纤维瘤（PN）。截图来源：CDE官网朗格汉斯细胞组织细胞增生症（LCH）是组织细胞肿瘤的亚型之一，已经被收录至《中国第一批罕见病目录》。组织细胞肿瘤是指起源于单核吞噬细胞系统或向单核吞噬细胞系统细胞分化的肿瘤。它是一种以MAPK信号通路激活为主要特征的克隆性血液系统肿瘤，目前手术治疗、放化疗、骨髓移植等治疗手段的疗效十分有限，患者预后仍然较差。MEK是MAPK信号通路的一个重要组成部分，具有促进细胞的生长、分化、增殖等活动的作用。肿瘤患者MEK的活性高度增强会使肿瘤细胞无限增殖和生长，在许多肿瘤的治疗中，通过抑制MEK的信号通路可以抑制RAF/MEK/ERK通路，有望抑制肿瘤的生长发展。因此，MEK被认为是一个非常重要的治疗癌症的靶点。MEK1/2抑制剂的作用机制是通过抑制MEK蛋白的活性，从而影响MAPK通路（丝裂原活化细胞外信号调节激酶），最终达到抑制肿瘤细胞增殖的效果；其还可以调节细胞周期蛋白和细胞凋亡相关蛋白的表达，诱导肿瘤细胞死亡。芦沃美替尼正是由复星医药自主研发的创新型MEK1/2抑制剂，拟主要用于晚期实体瘤、I型神经纤维瘤、组织细胞肿瘤、动静脉畸形等治疗。根据公开资料，芦沃美替尼用于治疗儿童朗格汉斯细胞组织细胞增生症已经于中国处于2期临床试验阶段。在2024年的欧洲血液病协会（EHA）年会上，研究人员公布了芦沃美替尼治疗成人组织细胞肿瘤的2期临床试验数据。在29例患者中，独立评审委员会（IRC）和研究者根据PET反应标准（PRC）评估得到的确认的ORR分别为82.8%（24/29，其中14例CMR、10例PMR）和75.9%（22/29，其中12例CMR、10例PMR），中位至缓解时间均为2.9个月。本次芦沃美替尼用于治疗儿童朗格汉斯细胞组织细胞增生症拟纳入突破性治疗品种，意味着这款产品有望在不久的将来为这种儿童罕见病带来新的治疗选择。参考资料：[1]中国国家药监局药品审评中心（CDE）官网.
